Thank you for your interest in starting a chapter of Sigma Phi Alpha at your institution! Complete the petition application below to be reviewed by the SPA Board of Directors.

Before you proceed, please note the following requirements:
  • Your dental-related program must be accredited by the American Dental Association (ADA) Commission on Dental Accreditation (CODA).
  • A charter fee of $100 must be submitted for your petition to be considered complete. If your petition is denied, a full refund will be issued.

We recommend that you review the fields below to ensure you have all the necessary information, then pay your $100 charter fee before completing the form. You can pay your charter fee after submitting the form; just make sure you don’t forget the final payment step!

Accreditation status determined by the American Dental Association Commission on Dental Accreditation:
List for each program, if more than one.
List contact information (name, email, phone number) for 2–3 individuals who will be designated as charter members.
